The theme seems to suit the composer of the Sonata in B minor
but
5 is Hubay
7 is Monti of Czardas fame
the rest you can work out for yourselves I give no more clues

1, 2, 5, 7 and 11 all correct. The Bloch here is Jozsef rather than Ernest.
